Diabetes and Safety Footwear: Why the Boot Can Be the Injury (2026)
Research on industrial workers who wear occupational safety footwear found that among those with diabetic foot ulcers, 58.9 percent had ulcers on the dorsum, the top of the foot. That is the opposite of the usual pattern, where diabetic ulcers appear on the sole, and it points directly at the boot.
The same study found 81.3 percent had forefoot ulcers and 31.8 percent had recurrent ulceration. The authors noted that until then there had been no focused studies on foot problems in people with diabetes who are required to wear occupational safety shoes, which is a striking gap given how many people that describes.
Why a safety boot behaves differently on a neuropathic foot
Peripheral neuropathy reduces sensation, and that single fact changes everything about how footwear should be chosen.
A worker with intact sensation gets continuous feedback. A seam rubs, a cap presses, a boot is half a size tight, and they feel it and adjust. A worker with neuropathy may feel none of it while the damage accumulates, which is how a boot that feels fine produces an ulcer.
The dorsal ulcer finding makes sense in that light. A safety toe cap is rigid and sits directly over the top of the toes. On a foot that cannot report pressure, the cap has hours to do its work before anything becomes visible.
There is a second complication most guidance misses. With neuropathy, footwear can feel larger than it is, which means a worker may size down and end up in a boot that is genuinely too tight. "It feels fine" is not evidence of a good fit here. Measurement and visual inspection replace sensation as the check.

What to look for, and what disqualifies a boot
There is no CSA category for diabetic footwear. Certification is performance-based, so the task is finding a boot that is properly certified for your site and has the right internal geometry.
Features that matter: a genuinely wide toe box so the cap does not press on the toes, extra depth to accommodate an orthotic, a removable insole, a smooth interior without harsh seams, a padded rather than rigid heel counter, and adjustable closure that can be loosened as feet swell across a shift.
Features that disqualify: narrow toe boxes, non-removable insoles, internal stitching across the toes, ultra-stiff heel counters, and anything requiring a break-in period past initial comfort.

For a neuropathic foot, a boot that needs breaking in is a boot causing damage the wearer cannot feel. It has to be comfortable from the first wear.
Cap shape matters too. An asymmetrical cap that follows the natural curve of the foot puts less pressure across the forefoot than a symmetrical round one, and composite caps can sit slimmer than steel for equivalent protection. Boots worth trying, with a caveat
We are not going to call any boot diabetic footwear, because no such certification exists and the fit that works for you is an individual question. What follows are boots whose construction lines up with the features above, as a starting point for a fitting rather than a recommendation.

Moisture is a bigger problem than it looks
Diabetic skin is more vulnerable to maceration, the waterlogged softening that occurs when skin stays damp. Macerated skin loses structural integrity and breaks down under friction that intact skin tolerates, which is the same mechanism behind ordinary blisters and a much larger problem here.
Hours of damp feet meaningfully raise injury risk. That makes sock choice and boot breathability part of the clinical picture rather than a comfort preference.
Cotton is the wrong fibre, because it holds moisture against the skin. Wool manages moisture through the fibre and keeps working when damp, and wool work socks are a reasonable baseline. Look for seamless or flat-linked toe construction, since a raised seam is a pressure point on a foot that cannot report it.
Change socks partway through a wet or hot shift, and avoid tight elastic at the top that restricts circulation. Waterproof boots need breathable linings rather than simply being sealed, because a boot that keeps water out and sweat in has solved the wrong half of the problem.
Daily foot checks are the actual safety control
Because sensation cannot be relied on, inspection replaces it. This is the part of the article that matters most and the part no boot purchase substitutes for.
Check both feet every day, at the end of the shift, in good light. Look at the tops of the toes and the dorsum specifically, given where the research found these ulcers appear, as well as the soles, between the toes and around the heel. Use a mirror or ask someone if you cannot see the underside easily.
Any redness that does not fade, any blister, callus, crack, discolouration or wound needs prompt medical assessment rather than watchful waiting. In a foot with reduced sensation and reduced circulation, small wounds are not small.
Inspect the inside of the boot too. Run a hand through it before putting it on, checking for grit, a folded insole, a protruding seam or a worn lining. A neuropathic foot will not report a stone.

The Canadian accommodation question
Workers sometimes arrive with a note saying they should not wear safety footwear, and employers do not always know what to do with it. American guidance on this point does not transfer cleanly.
In Canada two obligations run at the same time. Occupational health and safety legislation requires the employer to protect workers from foot hazards, which does not simply evaporate. Human rights legislation imposes a duty to accommodate a disability to the point of undue hardship, and diabetes-related complications can engage it.
Those obligations are reconciled rather than traded off. In practice that usually means finding certified footwear that works for the individual, which is a fitting problem rather than an exemption. Where that genuinely is not possible, the conversation moves to different certified options, task reassignment, or engineering controls that remove the foot hazard from the work.
What it does not mean is a worker in ordinary shoes in an area with a foot hazard. That leaves them exposed to the hazard the requirement exists for, and it is not an accommodation.

Getting the fitting right
Fit at the end of the day, when feet have swollen, in the socks and orthotics you will actually wear. CCOHS guidance on safety footwear makes the same points, and adds that you should walk properly in the boot rather than standing in it.
Measure both feet and fit the larger. Then check the fit visually and by hand rather than by feel: is there room in front of the toes, does the cap sit clear of the toes, can you see or feel pressure marks on the skin after wearing?
Where you have access to a clinician, podiatrist or certified pedorthist, involve them. Where an ulcer history, deformity or high-risk diagnosis exists, therapeutic footwear prescribed and fitted professionally comes first, and a certified safety boot may supplement it for the days PPE is required rather than replace it.
