Wet cement burns skin without hurting at the time. It reaches a pH of 12 or higher, against skin that sits between 4 and 7, and the reaction produces little or no heat. A worker can have wet mortar against their skin for hours feeling nothing worse than damp, while the damage progresses underneath.

That delay is the whole problem. By the time it hurts, the burn is established, and second or third degree burns can follow relatively short exposures. Portland cement is estimated to account for a quarter or more of all work-related skin problems worldwide.

Why it does not hurt until it is too late

The chemistry is straightforward. Cement contains calcium oxide. Add water and it becomes calcium hydroxide, which drives the pH sharply upward. High alkalinity destroys tissue in the same way a strong acid does, and cement burns are as severe as acid burns.

What makes it different from an acid splash is the absence of a signal. The worker is often unaware of the process because it produces little or no heat, and there is no immediate pain to trigger a reaction. The reaction continues for as long as the material stays in contact.

Cement burns look like other burns once they appear: blisters, dead or hardened skin, and discolouration that can run to black or green. Because the onset is slow, they are routinely misread as ordinary irritation until the damage is significant.

Clothing is not protection here

This is the mechanism that catches experienced tradespeople, and it is worth being blunt about.

Liquid from wet cement penetrates clothing. A worker kneeling in mortar, or wearing trousers that soaked through an hour ago, has cement held against skin under pressure for the rest of the shift. Damp or wet clothing makes the exposure worse rather than better, because it holds the material in contact instead of letting it fall away.

The classic injuries follow that pattern. Knees from kneeling in fresh concrete. Ankles and lower legs from slurry running into boots. Wrists and forearms from material entering glove cuffs. Every one of those is a place where fabric traps the material against skin.

Boots need to keep slurry out rather than simply be waterproof. From irritation to a career-ending allergy

The acute burn is the visible injury. The slower problem is dermatitis, and it follows a progression worth understanding before it starts.

Irritant contact dermatitis comes from repeated contact with alkaline materials. It appears within hours in its acute form, and the chronic form develops gradually with indistinct borders and can persist for years after exposure ends. It starts as dryness and itchiness, then redness, cracking and pain.

Allergic contact dermatitis is the more serious outcome. It occurs in response to small exposures once a worker has become sensitised, and in cement it is linked to hexavalent chromium, which forms during manufacture and is released when cement is mixed with water. Chromate sensitivity also makes cement burns more severe.

The consequence is what makes this worth taking seriously early. Sensitisation is difficult to reverse, and a worker who develops an allergy to cement can be unable to continue in the trade at all. It is difficult, if not impossible, to predict who will develop skin problems based on experience or medical tests, so seniority is not protection.

What to do when contact happens

Speed matters more than technique, because the reaction continues while you decide.

Remove contaminated clothing, taking care not to spread material onto unexposed skin. Wash the area thoroughly with plenty of clean water. Continue flushing rather than a quick rinse, since the aim is to remove and dilute the material completely. Seek medical attention promptly for anything beyond superficial contact, and immediately where a large area is involved or where the material reached the eyes.

One caution about advice circulating online. Some sources suggest applying vinegar to neutralize the alkalinity. Flushing thoroughly with clean water and getting medical attention is the safer course, and anyone with a suspected cement burn should be assessed rather than treated on a job site.

Do not wait for pain as the trigger to act. The absence of pain is a feature of this injury, not evidence that nothing happened. Rinsing after every contact, throughout the shift, is a habit worth building into how the crew works.

Skin left dry, cracked and irritated is also a barrier that has already failed, which makes further exposure worse. Washing with a pH-neutral cleanser at the end of a shift, rather than aggressive site soap, matters more than most crews assume.

Silica: the other masonry hazard

Cutting block, brick and stone is a separate exposure with a separate control, and it needs to be addressed alongside the skin question rather than instead of it.

Respirable crystalline silica is released when masonry materials are cut, ground or drilled, and cement itself contains it. Wet cutting and vacuum-assisted tools reduce the dust at source, which is where the control belongs.

Where respiratory protection is required, dust calls for a particulate filter rather than a cartridge, and a disposable is generally not the right answer for sustained cutting. Knees, and the exposure that combines both

It is also mechanically damaging in its own right.
